ANTERIOR MAXILLARY GROWTH IN IMPLANTOLOGY
Complications, Management and Prevention?
Pr ANDRE P. SAADOUN DDS, MS

Osseointegration techniques initially concerned only totally edentulous patients, then they gradually involved partially edentulous patients and finally, they developed enormously for patients with single edentulousness on healed ridges, then towards the immediate placement of the implant after extraction.

Consequently, it was becoming normal to place implants in adolescents with congenital agenesis and in accident-prone adolescents or young adults under the age of 30 after the failure of any conservative dental treatment.

However, it took more than 20 years through the publications of the literature to realize that this treatment modality caused several complications induced by maxillary/mandibular anterior growth in these young patients.

The effects of maxillary growth years later on implant treatment are unpredictable, but in general it can lead to complications such as
- An Infra-occlusion position of the Implant
- Peri-implant bone loss around the implant
- A Soft Tissue Recession on the Vestibular part of the Implant Restoration
- Infra Bony Defects around neighboring teeth
- Disorders of occlusion with all their Functional Consequences
- A Prejudice in the Aesthetics of the Smile

There is therefore a major risk, when this implant is placed on young patients between 15 and 25 years old, and less apparent complications after 30 years old

Clinicians should be aware of this phenomenon of continued maxillary growth after implant placement in the anterior region and inform their patients that these changes could

compromise esthetic results and adverse periodontal conditions. If the literature confirms the effectiveness of osseo-integrated implants for fully and partially edentulous patients, other forms of treatment should be considered carried out in adolescents and young adults, over a period of 15 to 20 years, replacing a single or several missing anterior teeth, and while waiting for more complete bone growth The purpose of the lecture is to show these complications, how to treat them and how today we could prevent them with a more appropriate treatment approach that is more specific, less invasive and using also the capacities offered by the Bonded Bridges by a Single Proximal Wing, while modifying the soft tissues of the edentulous unitary zone, by Bone Graft combined with a Connective Graft.
